Community Open House: Transitional Care at Sheridan Green House

Sheridan Memorial Hospital’s Transitional Care services will begin an exciting new chapter this June at the Sheridan Green House campus, bringing together two innovative models of care in a setting designed to support healing, recovery and independence.

The community is invited to tour the new Transitional Care facility during a public open house on Monday, June 8, from 4 – 6 pm in the Whitney Cottage at Sheridan Green House. The Transitional Care team will officially begin serving patients in its new location on Tuesday, June 9.

While the location is changing, the care patients receive will remain the same. The Transitional Care experienced nursing and therapy teams will continue providing the high-quality skilled nursing and rehabilitation services that have helped hundreds of patients recover and return home safely since the program launched in 2022.

“This move represents the best of both worlds,” said Kelly Lieb, Sheridan Memorial Hospital Director of Care Continuum and Experience. “Patients will continue receiving the same exceptional rehabilitation care from the same dedicated team, while benefiting from an environment intentionally designed to feel more like home. We are excited to welcome patients into a space that supports both healing and quality of life.”

Patients recovering from surgery, illness or injury will have access to private rooms and comfortable living spaces designed to promote independence and rehabilitation in a home-like environment. The relocation is part of Sheridan Memorial Hospital’s ongoing commitment to meeting the evolving healthcare needs of the community while enhancing the patient experience.

During the June 8 open house, community members will have the opportunity to tour the new space, meet members of the care team and learn more about the services available through Transitional Care at Sheridan Green House.

The open house will be held from 4 – 6 pm on June 8 at Sheridan Green House, located at 2311 Shirley Cove. Light refreshments will be served.
