Health Information Management
Health Information Management, also known as “Medical Records,” is a vital part of your patient experience. Once you leave our facility it is our honor to continue to serve you by providing your information timely and as requested. For your privacy and protection against medical identity theft we ask when requesting medical records:
- Please complete the Authorization to Release Medical Information Form. Give as much detail as possible. Please provide a phone number where it is most convenient to reach you if we need additional information. If faxing or mailing the authorization please provide two forms of ID. A photo ID is required (driver’s license) and at least one other form of ID such as a copy of your social security card or birth certificate.
- You may fax the information with a cover sheet to the number provided on the form 307-675-6762 or mail it to: Sheridan Memorial Hospital Health Information Management 1401 West 5th Street Sheridan, WY 82801
You can also bring the form in to our office on the lower level of the hospital. As of April 22, 2019, our Health Information Management staff are all located at the hospital – they are no longer at our downtown location.
If we have the pleasure of seeing you in our office we will ask for your driver’s license or other form of ID to verify your identity. If you are a parent asking for information regarding a minor child, a photo ID of the child is not required- just a photo ID of the parent or legal guardian.
Health Information and Records does not keep birth certificates in the medical record. Birth certificates are the property of the State of Wyoming. It is unlawful for us to provide you with any of their forms that are in the medical record, such as the mother’s worksheet or paternity papers. That information must be obtained through the State.

The cost for a copy of your birth certificate is currently $13.00. You may find the form to request your birth certificate at https://health.wyo.gov/admin/vitalstatistics/certificates/ . You will see a list on the left side of the web page, go to “Certificates- Birth, Marriage and Divorce. Make sure the form is sent to the Vital Statistics address given on the form. Typically requests take approximately 1 week to process.
