Updated April 12, 2021 – COVID-19 Vaccine Information
The list of people eligible to receive the COVID-19 vaccine now includes persons age 16 and over—individuals who are 16 and over are eligible to receive the Pfizer vaccine. While SMH is currently administering the Pfizer vaccine, the Moderna and Johnson & Johnson vaccines are available for persons over 18.
We now have two options for scheduling your vaccine appointment:
- Online – Click the box to the right to schedule your appointment online.
- By Phone – Call 307.675.4471 and leave your name, date of birth and phone
number to schedule by phone.
Please note, it is important to schedule your COVID-19 vaccination appointment at least 2 weeks after any other immunizations you may have received.
There is no charge for the vaccine, however, if you have insurance, your insurance company will be billed $40 per shot for an administration fee. Based on State of Wyoming rules, individuals will not be billed for fees associated with the COVID vaccine.
