Case Management Team
When you or a loved one need support upon leaving the hospital, our Case Management & Social Services Team is here to assist you. Some of the services available through our Case Management Team include: home health-care, hospice, referrals to community agencies, nursing home placement, clarification of benefits, and Advance Directives. This team is dedicated to helping you or your loved ones transition from the hospital to home successfully.
If you are admitted as an inpatient to the hospital, some or all of your care may be managed by a member of our Hospitalist Team. This is a multi-disciplinary team made up of physicians, physician assistants and nurse practitioners who are based at the hospital and are available seven days a week, 24 hours a day to provide you with expert patient and family-focused care throughout your acute illness or post-surgical stay.
While in the hospital, diagnosing your medical condition as quickly as possible is a top priority. Quality laboratory testing is available, and our pathologists are experts and work closely with primary care physicians, sub-specialty physicians and the Hospitalist Team to provide you with meaningful information in a timely manner.
Imaging Services Available
Within our radiology department, you will find a full service medical imaging department and our Radiology Team provides you with expert imaging services. Some common imaging services available include:
- Chest X-ray
- Lumbar Sacral Spine X-ray
Sheridan Memorial Hospital offers full-service laboratory services to you while in the hospital and outpatient lab services in the Outpatient Center Lab. Some common lab services you may receive are:
- Urine pregnancy test
- Strep screening
- Testing for the influenza virus
The Outpatient Lab is located inside the Hospital’s Outpatient Center, located at 1333 West 5th Street Suite 111 and the hours are Monday – Friday 6:30 AM – 5:00 PM. Contact the Outpatient Lab at 307.672.1035.
