During a recent appearance on Sheridan Media’s Public Pulse, Sheridan Memorial Hospital’s Dr. Kristopher Schamber and Director of Patient Care Continuum and Experience Kelly Lieb discussed the recent relocation of the hospital’s Transitional Care Unit (TCU) to Sheridan Green House and what the change means for patients and families
The pair explained that Transitional Care serves patients who are no longer sick enough to require hospitalization but still need skilled nursing, rehabilitation and support before safely returning home. Services often include physical, occupational and speech therapy, with a focus on helping patients regain strength, independence and confidence following illness, injury or surgery.
Lieb and Schamber highlighted how the move to Sheridan Green House allows patients to recover in a more home-like environment while continuing to receive the same high-quality care from the same nursing, therapy and support teams. The new location provides opportunities for “functional rehabilitation,” helping patients practice real-world activities they will encounter when they return home, from cooking and gardening to safely navigating daily routines.
Throughout the conversation, both leaders stressed the importance of maintaining independence, supporting Sheridan County’s growing older adult population and creating a seamless continuum of care that helps patients navigate the healthcare system more effectively. While the Transitional Care Unit’s location has changed, they reassured listeners that the people, services and patient-centered approach that have made the program successful remain the same.
